Abstract

Ortopedia dentofacciale per la gestione tridimensionale delle II classi scheletriche iperdivergenti

Skeletal Class II high angle patients are often associated with openbite, protrusive profile, lip incompetency, gummy smile or/and TMD symptoms, which are considered most complex situations in clinical orthodontics. Orthodontic treatment with premolars extraction has been applied to overcome/camouflage Class II malocclusion for many years, but TADs has been highly helpful in resolving transverse, antero-posterior (AP) as well as vertical problems in the recent years. Full arch distalization of the whole maxillary arch using TADs corrects Class II relationship into Class I occlusion (AP). In addition, simultaneous vertical correction such as full arch intrusion, segmental intrusion (vertical) as well as skeletal correction including maxillary expansion, clockwise or counterclockwise rotation of the mandible (dentofacial orthopedics) are also possible during the distalization through adjusting the line of force application. However, given that TMJ stability may not always be guaranteed in Class II high angle patients, orthodontists should be aware of the risks and benefits of dentofacial orthopedics. This lecture will provide an overview of diagnosis, treatment options and selection criteria for the non-surgical improvement of adult Class II high angle malocclusion with TADs. Clinical decisions and managements to control 3-dimensional tooth movement will be shared with treatment outcome and prognosis.
